== Molecular problem: fibrosis == When faced with pressure overload, the heart attempts to compensate with a number of structural alterations including hypertrophy of cardiomyocytes and increase of extracellular matrix (ECM) proteins. Rapid accumulation of ECM proteins causes excessive fibrosis resulting in decreased myocardial compliance and increased myocardial stiffness. The exact mechanisms involved in excessive fibrosis are not fully understood but there is evidence that supports involvement from local growth factors FGF-2, TGF-beta and platelet-derived growth factor. TGF-β1 plays an important role in cardiac remodelling through the stimulation of fibroblast proliferation, ECM deposition and myocyte hypertrophy. The increase in TGF-beta 1 expression in a pressure-overloaded heart correlates with the degree of fibrosis, suggesting TGF-beta 1 involvement in the progression from a compensated hypertrophy to failure. Through an autocrine mechanism, TGF-beta 1 acts on fibroblasts by binding TGF-beta 1 receptors 1 and 2. Upon receptor activation, the receptor-associated transcription factor Smad becomes phosphorylated and associates with Co-Smad. This newly formed Smad-Co-Smad complex enters the nucleus where it acts as a transcription factor modulating gene expression. Cardiac remodelling of the ECM is also regulated by the CNP/NPR-B pathway as demonstrated by the improved outcomes in transgenic mice with CNP over-expression subjected to myocardial infarction.

Unfortunately, when C3435T is mutated with a mutation from either exon 12 or exon 21 (or if all three mutations occur at the same time creating a haplotype), the inhibitors are less likely to weaken the function of MDR1. Multiple silent mutated genes tend to be more resistant against these inhibitors. Looking at the molecular level, the reason why C3435T in exon 26 of MDR 1 gene is not silent is because of the pace at which the amino acids are being translated to proteins. mRNA's secondary structures can fold which means different codons correspond to different folding's of the mRNA. For example, when exon 26 changes ATC to ATT both codons produce the same amino acid but ATC is seen more often than the mutation codon. As a consequence, the amount of time it takes for the ribosome to produce its protein confirmation is changed. This leads to a protein structure different from the usual shape of the protein which leads to different functions of the protein. Other reasons behind MDR1's "silent mutation" occurs in messenger RNA. In mRNA, codons also work as exon splicing enhancers. Codons decide when to cut out introns based on the codon it is reading in mRNA. The mutated codons have a higher risk of making a mistake when splicing introns out of the mRNA sequence leading to the wrong exons being produced. Libya had long supported the FROLINAT militia in neighbouring Chad, but FROLINAT became divided over its ties to Libya in 1976. In January 1978, the anti-Libya faction within FROLINAT, led by Hissène Habré, switched sides and allied with Chadian President Félix Malloum. Meanwhile, the pro-Libya faction within FROLINAT, led by Goukouni Oueddei, renamed itself People's Armed Forces (FAP). In December 1980, Gaddafi reinvaded Chad at the request of the FAP-controlled GUNT government to aid in the civil war; in January 1981, Gaddafi suggested a political merger. The Organisation of African Unity (OAU) rejected this and called for a Libyan withdrawal, which came in November 1981. The civil war resumed, and Libya sent troops back in. In 1982, the GUNT government was overthrown by Habré's forces and Oueddei fled to Libya, where Gaddafi provided him with arms to continue to guerrilla war against Habré. In November 1984, Gaddafi met with French President François Mitterrand; both agreed to withdraw from Chad. Oueddei broke with Gaddafi in 1985 due to the former's intentions to negotiate a truce with Habré. Consequently, he was placed under house arrest by Gaddafi and allegedly arrested by Libyan police and shot in the stomach. Oueddei survived the shooting and fled to Algeria, but continued to claim he and Gaddafi enjoyed a good relationship. When Gaddafi ordered the remnant of GUNT to attack Habré in February 1986 in violation of his agreement with Mitterrand, France launched Operation Épervier, which escalated into the Toyota War.
2C (2C-x) is a general name for the family of psychedelic phenethylamines containing methoxy groups on the 2 and 5 positions of a benzene ring. Most of these compounds also carry lipophilic substituents at the 4 position, usually resulting in more potent and more metabolically stable and longer acting compounds. Most of the early 2C drugs were developed by Alexander Shulgin in the 1970s and 1980s and were reviewed in his 1991 book PiHKAL (Phenethylamines I Have Known And Loved). 2C-B is the most popular of the 2C drugs. The prefix "2C" comes from the fact that they are phenethylamines rather than amphetamines and have two carbon atoms in their side chain, whereas the suffix (e.g., the "–B" or "bromo" in the case of 2C–B) refers to the substituent at the 4-position.

Doping existed in other countries, says the expert Jean-Pierre de Mondenard, both communist and capitalist, but the difference with East Germany was that it was a state policy. During the high-profile Berlin Doping Trials of the late 1990s and early 2000s, German prosecutors singled out the Sportvereinigung Dynamo (English:Dynamo Sports Club) as a center for doping in the former East Germany. Many former club officials and some athletes found themselves charged after the dissolution of the country. Victims of doping, trying to gain justice and compensation, set up a special page on the internet to list people involved in doping in the GDR. State-endorsed doping began with the Cold War of 1947–1991, when every Eastern Bloc gold represented an ideological victory. From 1974, Manfred Ewald, the head of East Germany's sports federation, imposed blanket doping. At the 1968 Summer Olympics in Mexico City, the country of 17 million collected nine gold medals. Four years later the total was 20 and in 1976 it doubled again to 40. Ewald was quoted as having told coaches, "They're still so young and don't have to know everything." In July 2000 Ewald received a 22-month suspended sentence, to the outrage of his victims. Often, doping took place without the knowledge of the athletes, some of them as young as ten years of age.

Gulf War veterans found evidence of a causal link between GWS and exposure to low levels of the nerve agent sarin, which was released into the air by coalition bombing of Iraqi chemical weapons facilities. Significantly, the study found an increased incidence of GWS not only among veterans who recounted hearing nerve agent alarms, but also among veterans with the RR or QR (as opposed to the QQ) forms of the PON1 gene, which produces an enzyme that deactivates organophosphates (including sarin) through hydrolysis. By contrast, GWS was inversely associated with higher levels of the type Q isozyme, which is more efficient at breaking down sarin than its type R counterpart. The authors "found that the PON1 genotype and hearing nerve agent alarms were independent and the findings robust to both measured and unmeasured confounding, supporting a mechanistic [gene–environment] interaction. ... Moreover, the change in the combined effect from one category to the next was significantly greater than the sum of the independent effects of the environmental exposure and the genotype".
== Toxicity == The lethal dose of amanitoxins is 0.1 mg/kg of body weight of humans. The average Amanita mushroom contains 3–5 mg of amanitoxins, so one 40–50 g mushroom could kill an average adult. The U.S. Occupational Safety and Health Administration (OSHA) permits a time-weighted average exposure of up to 5 mg/m3 of β-Amanitin dust.
